In consideration for and as a condition of my participating in activities provided by Underworld Laser (“ Underworld”) I hereby acknowledge that:

The activities I will engage in at Underworld are activities undertaken for the purpose of recreation, enjoyment and leisure and such activities involve a degree of physical exertion and the risk of physical harm.
Any such injury may result not only from your actions but also from the action, omission or negligence of other participants.
The statement set out in paragraph 2 above constitutes a risk warning pursuant to the Civil Liability act 2002 and I willingly agree to assume all risk in participating in the activities provided by Underworld Laser
In the event that I feel I, or any person under my care or supervision, is in difficulty during participation in the activities, I will stop the activity or request that the activity be stopped and seek assistance and/or advice from UnderworldLaser staff.
I will obey all signs and follow all safety directions given by UnderworldLaser staff or communicated through an instructional video on how the game is to be played.
I am medically fit (both physically and mentally) to undertake the activities and I am not intoxicated or under the influence of any drug or medication that may affect my mental perception, alertness, ability to learn or to respond to an emergency situation.
I acknowledge that during all times while I am participating in the activities I do so at my own risk and I do not hold Underworld Laser, the parent company Wildcard Entertainment Pty Ltd, or its employees or agents liable for any injury, loss or damage including personal injury that I may suffer while at or within the premises of Underworld Laser as a result of participating in the activities or otherwise.
I acknowledge that the activities may require me to assume a role involving responsibility for the safety/supervision of others participating in the activities. If an injury occurs as a result of any act, omission or negligence on my part then I assume full responsibility for any injury to the person and I indemnify Underworld Laser, its employees and agent in respect of any loss or damage incurred by that person.
I acknowledge that any offensive or destructive behaviour, be it physical or verbal, to either a person or equipment may lead to staff expelling a player or players from the arena and/or venue.
I acknowledge that any wilful damage to any equipment or property will see any costs incurred by Underworld Laser to remediate the damage passed on to the person who enacted the damage or their parent/guardian.
I have carefully read and understood this declaration, waiver and release and having done so sign same voluntarily.
